Start off by creating a new document 400px x 400px. Get the rounded marquee tool and set the foreground to black.
Create a new layer and make a selection of a perfect circle (hold shift while making the selection). Fill the selection with black.

Make a selection in the center of the circle and press delete.

Remove from the circle so you will have a ¼ of what you had.

Duplicate the layer we worked on. Press CTRL + T to bring up the Free Transform menu. Resize the shape so it will be bigger than the one we created. Marge the layers together. (Select both layers > Right click > Marge layers)

Duplicate the layer with both shapes and get the Free Transform menu (CTRL + T). Rotate the shape and move it so it looks as in the image below:

Create a perfect circle in the center.

You would also like to smooth the edges, to do this you will need to marge all the layers (with the background) and apply a Gaussian Blur of 8 (Filter > Blur > Gaussian Blur). Press CTRL + L and bring all the 3 arrows as in the image.

You can add a text and you are done !
